First, there is always the fallback option of having multiple installs of the code. This would involve having several directories on your site, and pointing each form to the specific directory. Each controls.php file could be configured differently.
The downside is that this is redundant, requires you to upload the same files multiple times, etc.
So, as of 3.2.8, here is the new method:
Upload multiple copies of controls.php to the main Ultimate Form Mail server directory (ufm by default). Rename them in one of two ways:
controls-1.php
controls-2.php
controls-3.php etc
or
controls1.php
controls2.php
controls3.php etc
*very important*
Your main, default configuration will be in the original controls.php file. You MUST have a file named controls.php to make sure everything works properly.
Next -
In your opening form tag, of your html (contact page) you would modify the "action" slightly like so:
form action="/ufm/mailit.php?cid=2" method="post">
the ?cid=2 means 'controls id #2' and would grab either controls-2.php or controls2.php (don't have both, just one).
Questions -
What if controls-2.php isn't found?
It defaults back to controls.php, which is why you need to have a controls.php file on your server in the ufm main directory.
What if leave out the cid=X ?
Again, it defaults to the controls.php file. So make sure it's available to be found.
Do I have to use this feature?
No.
Do I have to number my controls.php files in order?
No. You can use whatever numbering system you want, but it must be numbers only.
What if I have a field named 'cid'... will there be a conflict?
There shouldn't be. It should work just fine.
Can I name my control files anything other than controls-X.php or controlsX.php ?
No. Maybe in a future version, but I really don't see any need to make this change.
I would like to have one form with two or more pages. The second page and quite possibly more would be dependent on an option (radio button) or drop-down menu selection. Then the third page would be dependent on another selection. Can this currently be done? If not, is it something that would be considered in the future? I have a customer who wants this type of form and I'm in the process of developing the web site.

Is their a way to add the name of the person filling out the form in the
webmaster_subject area?

The customer want the name from the required field 'name' to appear in the subject line
I can't find anything at the support and the system will not let me add
a tickets keeps telling me to look in the knowledge bas and I have there is nothing there.
Subject line in email from this:
Mercury Jersey Order
To this:
Mercury Jersey Order - My Name Here
